PLEASE NOTE:  Once you have completed successfully the course final exam, which is composed of fill-in-the-blank questions and multiple choice questions, the State of West Virginia requires students to complete an original final exam.  The school needs to mail you the application to the West Virginia written exam.  Here is a sample of the application for the student to review.
